Overview
Registering for MyPatients is a three-step process for referring providers.
- Be sure to have your NPI (National Provider Identifier) number handy.
- At this time, MyPatients is available to the following provider types: MD, PhD, NP, PA, DMD, DDS, DO, and OD.
You'll need:
- A copy of a valid, government issued ID (U.S. Driver's License or Passport).
- A signed Certification & Agreement.
- A signed Sponsorship Agreement (if requesting Sponsorship Privileges).
You'll also need either:
- Access to your practice letterhead and your practice fax machine, or
- Access to a Notary Public.
Fax or E-mail:
- A copy of a valid, government issued ID (U.S. Driver's License or Passport).
- A signed Certification & Agreement.
- A signed Sponsorship Agreement (if requesting Sponsorship Privileges).
Once documentation is received, it will take up to 3 business days to process your request.
